Tips for creating an effective profile:

  • Be specific: Instead of "likes music," write "loves listening to classic rock, especially The Beatles"

  • Use positive language: Focus on strengths and preferences rather than deficits

  • Include the person: Whenever possible, involve the individual in creating their own profile

  • Keep it current: Update the profile regularly as preferences and needs change

  • Think practical: Include information that will actually help supporters in daily interactions

Using the completed form:

Once completed, this profile can be:

  • Shared with new support staff or caregivers

  • Included in care plans and educational documents

  • Posted in living spaces or work areas (with permission)

  • Used during transitions to new programs or services

  • Referenced during team meetings and planning sessions
